I haven't played in a few days, patched this morning and the game won't launch. Every time I try it patched again and crashes.

Let's start with basic questions, did the game ever work or is this the first time you've tried to launch it?
Have you tried this solution - Anyone that has issues (there's a few small ones) - direct download - https://realmserver.com/files/TheRealmOnline.zip @everyone
I have played a lot, on this computer. Just not in a week or so.
And my Wife also plays and she is getting the same crash I am.
Download a fresh copy of the game, uninstall the old copy and put the new one on. Make sure you run it as an admin. See if that fixes it. Sometimes the updates can mess a file up. A fresh copy usually fixes it.
The download link LilGoofsta posted fixed it for me.
