Stuck in Combat without combat menu wheel

When entering combat with another player (in a group), occasionally neither of us are given a combat menu wheel at the beginning of the fight and we have to alt-f4 to fix it. It seems to be focused on only one player, as long as that one player alt-f4 and can get their account to go link-dead, the battle can resume for everyone else in combat. The combat wheel appears for them.

We thought it might be caused by one player leaving the screen while another engages in combat. But it has happened even after we all confirmed before engaging. Any workaround to this would help, since it requires at least one player to always die to fix it.

We found an easy way to reproduce this. If we both start a fight at the same time, it will always cause this.
Also the group leader is never the player stuck. In other words, the group member is the focus and one that the text says "waiting for player: XYZ". And player XYZ has to alt+f4 to fix it for the group leader
